Program Coordinator
The Homeless Outreach Team seeks a Program Coordinator to oversee CAB’s Hospitals to Homes initiative.  The position will be responsible for the ongoing development of this initiaitve and duties will center around liaising with Bronx hospitals to coordinate services for street homeless clients who are chronic users of Bronx hospital emergency rooms.  The Program Coordinator will oversee services including, but not limited to, medical care, psychiatric evaluation, psychiatric care, discharge planning, emergency shelter or stabilization bed referral, and housing placement.  The position will also be responsible for facilitating case conferences between CAB, Bronx Hospitals, and other homeless service providers, and for providing training for hospital personnel in areas specific to the homeless population and housing placement such as housing applications, the housing placement resources available to chronically homeless individuals, and housing focused psychosocial evaluations.  The position requires LMSW but will accept someone with a Masters degree in social work on a provisional basis.  An LMSW must be obtained within 3 months of employment.  Experience in a hospital setting is strongly preferred.  Bi-lingual English-Spanish is also preferred. Retention Specialist/Case Manager
